Rosie O’Donnell is opening up about a deeply personal decision after revealing she underwent a facelift earlier this year, sharing unfiltered before-and-after photos while reflecting on aging, self-image, and personal choice.
The 64-year-old comedian and former talk show host disclosed in a recent Substack essay that she had a lower deep-plane facelift, a procedure she said cost “more money than I have ever paid for a car.”
A Change She Once Thought She’d Never Make
For years, O’Donnell said she strongly opposed cosmetic surgery and viewed facelifts as incompatible with her beliefs about aging and feminism.
“I thought it was a betrayal. Of feminism. Of aging. Of our team of women worldwide.”
However, after losing around 50 pounds, she found herself reconsidering her stance and began researching the procedure.
Her Daughter’s Reaction
O’Donnell revealed that her youngest daughter, Clay, questioned the decision when she learned about the planned surgery.
“Young women look up to you.”
The comment gave O’Donnell pause, prompting what she described as an internal debate about beauty standards, authenticity, and personal freedom. Ultimately, she decided to move forward with the procedure while maintaining that cosmetic choices should remain a personal decision.
‘Not One Person Noticed’
One of the biggest surprises, according to O’Donnell, was the reaction after the surgery.
“Not one person. Not a friend, not a stranger, not even people who owe me compliments.”
She joked that after going through what she called a “full existential feminist crisis” and surgically altering her face and neck, the result was that almost nobody noticed.

Reflecting on Aging and Choice
Despite previously saying she never planned to undergo plastic surgery, O’Donnell said she remains comfortable with her decision and views it as part of a new chapter in life.
“For the girl I was. The woman I am.”
The actress has previously spoken about embracing natural aging and believed it would help her land more mature acting roles. Her latest comments highlight how personal views can evolve over time, even on issues people once felt strongly about.
